Using the Coalese Jakub Patient-Oriented Delivery Foundation, Raven can optionally provide cross-clinic queries and a consolidated view of a patient record across multiple clinics.

Cross-Clinic Query

Within a community of related clinics (such as a PCN or LINS), the combination of Coalese Raven and Coalese Jakub provides a community-level “virtual” EHR solution, with integrated access to consolidated patient records across the whole geographical community.

The ability to do cross-clinic access to patient records is also very beneficial to mobile healthcare workers, such as homecare nurses, EMT/Paramedics and others who routinely provide services to patients across a wider geographic community that may include numerous clinics. Raven Collaborative Access, in conjunction with the Jakub Patient-Oriented Delivery Foundation, enables mobile primary care team members to access patient records regardless of the worker's location or which clinic(s) a patient record(s) is located.
