Running Metrics and Performance Indicator reports using Coalese's Raven product employs a simple and intuitive user interface that requires no training. Bbecause there is no learning curve, physicians, clinic administrators and even PCN/LHINS/Regional staff can quickly produce full-features metrics reports....just log in and click:
Raven Intuitive User Interface - Metrics Report Request
The intuitive Raven Metrics Reporting user interface requires no training or education. Users become proficient with only a few minutes of using the tool. Advanced parameters are available with a single click for those users who need to further refine their reports or who have specialized reporting requirements.
Coalese Raven also includes role-based access controls, so that only personnel authorized by the clinic/physicians are allowed to run Metrics and Performance Indicator reports. The Coalese Advanced Metrics Management system can be accessed securely from any web browser.
Raven Intuitive User Interface - Completed Metrics Reports
Here is a sample of a Validated Patient List:
Raven Metrics Report Output - Sample Validated Patient List
Validated Patient lists can be created with a number of user-selected filtering algorithms, for example: AHW 4-Cut, Last Date Seen and EMR Assignment algorithms. Summary statistics include age-breakdown graphs and other useful totals and percentages. Display of the full Patient Medical Record using Raven's intuitive icon-driven, touch sensitive display is available with only a single click on the patient's name or healthcare number. The VPL Patient Attachment Exceptions report makes it quick and easy to validate which physician a patient is assigned to, thus keeping patient lists accurate, over time, in multiple-physician clinics.
These Validated Patient Lists provide a consistent basis for more advanced Chronic Disease Panel reports and metrics, for example: Hypertenions, Diabetes, Cervical Cancer Risk Screening and many others coming soon, with the Patient List being the "denominator" for such indicators. The Coalese Advanced Metrics Management System ensures that this "patient list" denominator is consistent over time and provides tools and related processes to easily keep the patient list up-to-date, thus enabling valid performance and diligence comparisons across prior periods over longer periods of time.
Coalese's Advanced Metrics Management System can also generate AHW (Alberta Health & Wellness) PDI (Performance & Diligence Indicators) spreadsheets in both HTML and Microsoft Excel formats with a few mouse clicks, thus making it easy for physicians to submit their validated patient lists to obtain government incentive payments. The HTML PDI spreadsheet reports also provide advanced sorting capabilities to facilitate the confirmation of patient assignments by listing patients who need to be contacted along with their contact information.
Other reports such as the Physician Deceased/Left Practice List provide physicians with useful tools to help manage their patient panels more effectively. By knowing which patients left a physician's practice during the prior period (typically a year), the physician can more easily determine how many "open slots" they have to accomodate new patients.
Coalese's Advanced Metrics Management System provides a wealth of progressive Best Practices/Chronic Disease Panel reports and metrics, including: Hypertenion, Diabetes and Cervical Cancer Risk Screening with many others, such as COPD, Cardiac Disease and Smokers @ Risk, coming soon. These reports quickly and easily identify patients that have or are at risk of contracting specific conditions, thus allowing the primary care team to institute preventative measures, which greatly improves patient welfare and reduces systemic healthcare costs dramatically. Integrated graphing and charting provides "at a glance" summary information to the practitioner.
The Coalese product uses sophisticated algorithms to determine if a patient has or is at risk of contracting a specific chronic condition. These algorithms evaluate observations, lab results, prescribed medications and also do advanced pattern matching on physican's free form text encounter notes to make such determinations. Many physicians using EMR systems enter the bulk of their key diagnostic and assessment information in free form text, and so Coalese has implemented multi-faceted pattern matching to extract diagnostic information from such sources. For example, the Coalese Raven product can easily detect blood pressure readings of the form 145/90 (systolic/diastolic) in free text and use these in it's reporting, along with matching of phrases that indicate positive, at risk or negative conditions for hypertension, diabetes and cervical cancer.
Here is a sample of a Hypertension HTML report:
Raven Metrics Report Output - Sample Hypertension Report
Reports can optionally include patient details, in addition to the summary statistics, which show various patient-level indicators, such as Smoking status, Cessation counselling and Blood Pressure as per the above Hypertension Metrics report example.
The Coalese solution takes a step beyond just identifying positive and negative indicators and diagnoses: it also identifies patients that are at risk of developing a chronic condition (shown in blue on the above sample report). This capability allows physicians to pro-actively assist in the management of their patient's health to emphasis prevention, thus improving patient welfare and reducing systemic healthcare costs. Taking steps to prevent "at risk" patients from developing debilitating chronic conditions such as diabetes, hypertension, cervical cancer and COPD holds out the promise of huge benefits to the patients, the healthcare system and society as a whole.
The user can also show the detailed reasons why a patient was flagged with specific indicators by simply clicking on the "Show Rationale" button beside the patient entry they wish to validate.
Here is a sample of a Hypertension Rationale page for a patient, created with a single click on the “Show Rationale” button from a Hypertension Metrics report:
Raven Metrics Rationale Output
The Rationale output example above shows the exact information that was used to classify a patient as having a particular metric indicator such as hypertension, smoking status, etc. For each indicator, the phrases which were matched, relevant observations such as measurements or labs and prescribed medications are displayed or highlighted in context to provide insight into why the Raven Metrics algorithms classified a patient into specific categories. This feature provides very powerful and productive information to staff charged with validation of the metrics report results, improving accuracy, boosting efficiency and reducing the costs of validation and report customization processes.
Due to the variable nature of how physicians compose their free-text encounter notes, some complex phrases could potentially be mismatched by the advanced pattern matching algorithms that are used by the Coalese Advanced Metrics Management system. For example, depending on how a physician phrased their encounter note, a patient could be flagged as being hypertensive when in fact they were not positive for hypertension. To make it easy for the medical practitioner to correctly classify patients, the Coalese Advanced Metrics Management system provides a streamlined and easy method to "Fix This". At the top of the Rationale display page, per the example HTN report shown above, key indicators have a "Fix This" link under them.
Raven Metrics - Sample Diagnosis Fix This Popup Window
Clicking on this hyperlink will open a popup window which will allow the user to alter the classification of a patient with a single click, as shown above. This approach, in keeping with Coalese Raven's intuitive ease of use approach provides an easy mechanism for clinic staff to maintain diagnostic indicators, with minimal impact on clinic workflow and time demands. Of course, diagnoses can be altered at any time, should a patient's status change, with a full historical chronology of such changes preserved and easily viewed using the Coalese Raven patient record viewer.
